1: According to this thread, B12M is described as «with P222B12M special downgrade version for 12MHz CPU, for tandem configuration only» (Firefox translation.)
2: From the same thread, we learn P231B is «the latest version for some old motherboards with 256K RAM», which seems to suggest that release requires 1 MB RAM installed.
3: I wasn't able to find solid information for P271x releases, thus far… One would assume they're newer, but the aforementioned thread shows P211x from -4 onwards, so I can't tell for sure, however, several threads mentions it as a fairly new thing, using 4.0x programming software, which is one of the newer ones, that thread also goes into more detail, but since it supports PRI, I'm going to assume it's -6 (Version 6) level.
For KX-TD816/KX-TD1232 there are at least three programs I know of:
There are possibly more, but those are the main ones. They connect to the unit via serial port and are somewhat dependant on the firmware version (the infamous Programmator, for example, probably messes some KSU's because it tries to change stuff in a way that's incompatible with certain firmware versions.)
DSHSOMT is not easy to find, but you'll find it if you're so determined. Also probably holds true for PPD, but I wasn't able to track any version of it. Programmator is out there, but I didn't bother with it.
This is the old dealer software, and like I mentioned before, it's old and runs on MS-DOS, FreeDOS or DOSBox, but at least it's official. The only way I was able to find it was by searching through Usenet and ending up at this thread in which the guy asked for donations and got trolled to the ground. But one guy linked the URL to the dealer software, that was, thankfully archived by Web Archive (I love this tool.).
I've also managed to track two more versions (3.41X and 4.02B2, respectively from circa 1998 and 2000, judging by the copyright notice.) to a Russian abandonware website called «Old DOS».

3.41 seems to tend to international market, and it even asks the region of the equipment upon start:
Again, my equipment has the P401A60905A ROM version. 3.41X had it's issues at the first moment, but it was the version that best worked for me. Not only it can display and change every configuration as it was also able to back up the configuration. Your mileage may vary, depending on region and ROM version. Since I got a TD816-1, there are certain things that the tool is capable of doing, but won't work because the firmware doesn't support it, like configuring the hotel application.
